After more than 3 weeks of use, the card is dead and not published, not recognized in the BIOS

The card had problems from the start, and when it was used, more problems arose, and after 3+ weeks of use, the card does not start in the bios and is not created. Tried several different machines and operating systems, Win 10 Pro and Win 8.1 Pro and Linux, the card doesn't work at all. Was the controller chip heatsink very hot to begin with? The air supply was more than sufficient. The card was not plug and play to begin with and the problems got worse over time and now the card is just dead and had to be removed from the system as the card failed the PCI BIOS check and at boot got stuck.
