The screw inside is broken.
We used this swivel on a disc swing (which holds 650 pounds) in our church playground. After 6 months of normal use (never exceeding the 800lb mark) the bolt inside it broke. Luckily the child wasn't hurt! We're replacing it with a 1500lb capacity. Hope it lasts longer!
