Failure after a few weeks

I installed these plates for my dishwasher and within 6 weeks we lost a screw and a pad fell out of the rack. It seems to be a general problem with this particular product. We bought this house two and a half years ago and had a similar setup to mount a countertop dishwasher when we moved in and it has been around for at least as long as we have lived here so I was shocked and disappointed when I saw it saw you fail so quickly.
