I highly recommend this extended pedal with floor extensions. This combination allows the rider to rest completely on the ground and it's much more comfortable. Installation required removing the skid plate/skid plate on my 20's Road Glide, which isn't that difficult, but the exhaust pipe was a little tricky to work with (without removing it), but doable. It took about 45 minutes to complete the pedal and floor plate extensions and I am very happy with the result. Using the rear brake is a little different: the pedal angle is flatter <to the horizontal> and requires a bit more foot movement, but it really is such an improvement. That's how it should be in stock. The pedal should have a new cotter pin, but there isn't one. Overall, it has greatly improved my comfort and driving experience.
