Doesn't fit tight and falls off

I've had several of these. It has a strap under the foot that cannot be stepped on. It's hard to stay on the boot unless you untie the laces and secure with a lace. Most of my previous ones didn't attach to shoelaces, but were held in place by the tension of an elastic strap. Because it doesn't have an elastic strap, it fell off my foot on a very busy freeway at 65 mph. Another bad product for me.
