After installation and reassembly, the latch knob was loose making it appear the entire handle assembly was loose. When I took it all apart again, the handle was securely attached to the door. There is a plastic pin that goes through the handle and the lock button that slipped out of the lock button. I don't know if there should be an adjustment nut or other means to keep the pin from slipping out, but they should have been. I came up with my own solution to keep the pin from slipping out and now everything works fine.
