I like this cervical collar and here's why. I have a degenerative spine with many complications and have already had a titanium fusion in my cervical spine. The cervical collar that I got from the medical supply store was the basic white kind you get which is really smooshy and doesn't give much support when you sleep. The ones from the drugstore are flattened into a box and never maintain a round shape so they are impossible to wear. This one is firm enough to give you support if you want to wear it throughout the day, but I feel it will be adequate to sleep in also. Will update my review after a few nights with it. My neurosurgeon told me that when you have cervical spine problems that you should turn your brace around so it fastens in the front. It gives you more support on the back of your head and neck especially if you need to look up-it does minimize that pain. I have a skinny neck. Every time I go to the emergency room and they have to put me in a collar they put me in a pediatric one. I ordered the medium and if it were any tighter I would have to send it back but I think it's going to work for me. I hope this helps someone.Update: I have now slept in my neck brace for five consecutive nights and it was awesome. It's soft enough that it doesn't interrupt my sleep. If anything it keeps my neck so stable that I slept better. I spent the entire day before Thanksgiving and most of the night before in the emergency room finding out that I have another protrusion in my spinal column that is pressing into my spinal cord and I'm going to need another neurosurgery and another fusion so I am very very grateful for this brace. I will wear it before and after my surgery until it wears out and then I will order another one for myself. Until then my husband has been wearing this one around the house if I take it off at all and has fallen in love with it so I'm going to order him one also. He too has spinal issues just not as severe as mine. He is also skinny so he likes the medium as well. If you have a very muscular or thick neck I don't think the medium will work for you. (I bought the comfort one as it is softer for sleep).
