I was working for Cigna as my first job after college when they moved their entire system from paper medical records/paper claims processing into electronic format (i think it's called Epic).
They implemented this new platform very quickly but unfortunately had lots of bugs at launch that caused some issues before we could get everything iron-clad right away! It took many months until things were 100%. At least one year passed between each release so there would be something broken or missing somewhere along those years - especially around holidays where companies tend send out patches less frequently than usual due to fewer resources being available during holiday weeks / summer vacations etc..
