I have severe back pain and am excited to try this remedy. It's made of hard, dense foam with a smooth, easy-to-wipe surface. The surface is shaped with rounded channels that "give" slightly under pressure, but are not technically "soft". The instructions are a bit vague, but I managed it through trial and error. You can move and twist it in different directions to change the stretch in different areas of the spine. It can also be used while seated, but I've found it's much more effective when gravity pulls me towards it. And yes, there were a lot of cracks in my spine, which I think is a good thing. No pain but I reduced the session to 10 minutes. I really like it but I wouldn't pay $40 for it. Maybe $20.
