The shank came loose from the wheel pad

The polishing pad is made of good material and worked well until after about an hour the drill shank came loose and twisted in the pad rendering it unusable. - To replace the factory shaft, I pulled it out, drilled a hole through the center of the pad and installed a 3/8" and 3" button head screw with anti-rotation washers on either side of the pad. I used a nut on the side of the pad opposite the bolt head to tighten the bolt to hold the washers in place and countersunk the bolt head to avoid scratching the polished finish, although the wheel works best, when using the edge of a polishing wheel. Now it works great, I can put as much pressure on the wheel as I want and the pad no longer slips on the shaft.
