I bought the smaller version which doesn't come with a fan. I was glad they eventually added extra pin extensions on the side so you no longer have to solder your own pins to replace those covered by the cap (it covers 6 GPIO pins - 3V, both 5 V, ground and both serial clock pins). This makes it easy to add your own 3.3V or 5V fan, and you should, because as others have pointed out, it gets pretty hot. The smaller version fits the CPU heatsinks, but to fully install it you'll need to remove any heatsinks that may be on top of the RAM and Ethernet chips - as long as you're using a decently ventilated case with a fan it doesn't matter. One last note, it's rated for 2.5A, while the Pi 4 B is rated for a 3A power supply. Usually, this only matters if you're fully loaded with USB ports. If you plan to use this and your Pi as a POE network device (PiHole, HASS, etc.) then you're fine. In this case, you will hardly go beyond 1.6a.
