I wear 11/11.5 on NB and Asics so I ordered 46, removed the OEM insoles and added a good aftermarket replacement ($12). , so that the insoles fit perfectly ! I do this with all shoes. No more tingling in your toes or pain in your feet. After about 10 hours of riding, including 3 rides of 2 hours or more each, including some steep climbs in loose terrain, I can highly recommend this shoe for mountain bike road cyclists. The sole is fairly stiff, minimal flex (I don't feel it on 187) but no heel lift when walking. I ride SPD 100% and unlike the other big brands all 3 straps are close to the outside so there is no need to cut the middle strap so it doesn't rub on the grip. With proper cleat placement, there is also no crank friction. Aside from the lack of a marked template for precise cleat placement, I give these shoes a 4.7, rounded up to a 5.
