Most of my ESP32 circuits are normally inactive, so no heatsinks are needed. I recently built a circuit using a generic ESP8266MOD D1 Mini driving 8 strings of 50 RGB LEDs through rich visual patterns. The 7805, which supplies 5V for a miniature device with a regulated 12V supply, got noticeably warm, not very hot but annoying nonetheless. Screwing in one of these heatsinks and a gasket with the enclosed screw, washers and insert was done quickly. After that, the radiator/controller is barely warmer than skin temperature.
