The lid handle is falling apart, inside a piece of metal got into the food, dangerous!

I found round metal three days in a row while serving the soup. It turned out that the handle of the lid is falling apart. Can you imagine if someone accidentally swallowed a 1.5 inch round metal? I've only been using these pots for four weeks. This is really bad quality and dangerous.
