Terrible leaking right after installation

I've been installing plumbing for years, but this leaked from the start. I used teflon tape, changed the washer, tried to tighten it and not that tight but ended up throwing it away. The reason I went with this one was because it was more expensive and didn't want cheap alloy. So much for that train of thought.
