5'10, 165lbs I ordered small and medium due to medium fit. I usually wear a medium shade on Carhartt products and decided to just try both to see which one suits better. The little one fits well with just a bottle neck around the waist. It is not enough to cause problems when carrying a backpack during the hunting season. The medium also fits well, just a little looser. If you layer your clothes properly, it works great! If you plan on doing activities that will make you sweat or get wet, this still works well, but there are better options. I usually wear a classic polyester base layer when I know I'm going to be moving a lot, in the rain or in the water. Because it's an 81/19 polyester/wool blend, it dries much faster than regular wool. If you're looking for a good thermal, this will help. Reading other comments, I want to emphasize that this is just a base layer that needs extra layers to stay warm (clothing alone won't keep you warm).
