I installed this on a 2006 Accord V6 in early 2010. In the 2 years or 40,000 miles since then I've had no problems with it other than the pain of getting back into the wheel well about once a year to clean the filter. Installing the K and N wasn't too bad, but you have to jack up the car, remove the driver's side front wheel and remove the plastic wheel arch liner to get to it. Just make sure you don't break any of the little plastic clasps that hold it in place. Removing the OEM intake system sucks. It's definitely not meant to be pulled out, and it takes some serious dexterity and proper twisting/pulling to pull everything out. It took me about 3 hours to do all the work, 2+ of which was the disassembly and removal of the OEM. K and N are screwed directly to existing holes. Also, since it's so low on the car, make sure you use the dry charger, which fortunately comes with it. This is a type of protective tape for your filter and helps with water or debris that has splashed onto the filter through any holes. I noticed a smokier sound coming from the car, especially above 4,000 rpm and a few more ponies. I usually drive my car for efficiency, not performance, but I'd say I've gained maybe 0.5-1mpg, which should pay off in the long run.
