One of the two mounting nuts welded to the frame had the threads snagged because a large, thin, sharp piece of metal was jamming the threads and this screw couldn't catch. I was able to take a sharp flat head screwdriver, snap off the stray metal plate and install the screw. This is the first time I've seen this in 25 years. Such a poorly made nut thread.
