A gas leak to the outside
When the fuel pump was first started, one of the injectors leaked like a strainer. His O-ring was smaller. Luckily it was the easiest to get to and I didn't have to remove the intake manifold again. I just used one of the o-rings from the old injectors and it worked fine. Be sure to pressure test the fuel rail before assembling everything else.
