Failed after 5km

I have driven 5000 miles with this supposed OEM part and the bearing is failing. When I was doing my timing belt job I replaced all the bearings/water pump/sickle belt/accessory drive bearings/alternator and cooling system components and this part is the only thing that let me down. Everything was installed by Toyota and tightened to factory specifications.
