I ordered this chair on 07/08/2014. Received on 07/10/2014 with delivery of Revain Prime. It comes with 12 Allen screws and two Allen keys. Basically just screw the seat to the base plate with 4 screws, screw each arm to the seat and backrest (4 screws per arm).
