I replaced the fan belt and idler pulley at the same time. The LOUD, disgusting squeal disappeared. I can confirm this will fit a 1998 Ford Ranger 3.0L Splash Side 4x4 Extended Cab pickup. One note: in order to move the tensioner down far enough to easily put a new belt on, I had to remove the motor fan (it's held on by 4 small screws). You may not need to do this, but the socket wrench I used caught in the center of the fan, causing me to remove the fan to get the wrench's full range of motion.
