I should have just bought one of these along with the cheap aluminum heatsinks that came with my Pi. I'm using 3b+ with an OctoPi in a relatively warm room without high airflow and it's basically always on and typically streaming 720p from the webcam, on top of handling the load of Gcode transfer to the 3D printer, so I'm easily over 65 degrees went and experienced throttling fairly regularly. The Pi is wall mounted and housed in a 3D printed case designed to mount a 30mm fan like this one. The install costs $6 and is 5 minutes late and my CPU temp is now 40 or less. I don't know about other people experiencing noise, but even when connected via GPIO to a 5 volt power supply (instead of the quieter 3.3V or whatever in the product photo) I can literally do it from 10 inches away not hear. - because my Ender 3's audio is on even when I'm not printing and I don't have any stepper motor noise. 5/7 would recommend to a friend and will continue to monitor the life of this fan as it runs more or less 24/7 until it dies.
