Stop fooling around and buy this kit. I modified my shower tile to make the wall too thick so the valve stem and trim ring screws cannot be reached. This kit has a Crome pipe that extends an additional 1 inch. I only needed ¼ inch so I used a hacksaw and it was cut to size in five minutes. He tapped it with a hammer and put a brass valve extension on the old one. Now put on the slip ring. Don't worry if the new brass piece is a bit loose. A new, longer screw goes through both and holds the crappy plastic handle in place nicely. Then insert the longer screws into the two holes in the trim ring. This is actually the most tedious part - lining up the screws with the holes in the wall. But don't give up. You can do this. Comes with a nice chart.
