I replaced the original OEM belt tensioner on my 2003 Honda Accord V6 with this one from Dayco. It fits perfectly and seems to work well. My idler pulley (the one in the center of the tensioner) was making a lot of noise (loud howling/squeaking). I decided to just replace the entire tensioner which comes with new idler pulleys and idler pulleys. I removed the front right wheel and loosened the fender liner just enough to get it out of the way. When you remove the tensioner, the middle screw doesn't come all the way out, but it pops out far enough that the entire assembly can be removed after pulling out the bottom (and much smaller) screw. Fits, works, I'm satisfied.
