Horrible design renders this product inoperable

The pin that fits into the oxygen tank is too long, or the thread on the cap doesn't go deep enough. The result is the same if you try to connect it to an oxygen tank, the pin goes in too far so the gas pressure pushes it back so the thread for the regulator never reaches the cylinder thread. It cannot be clipped onto a disposable oxygen cylinder.
