Set screws are located along the center line and may cause the track to wobble even on a flat surface. The flat nuts under the guide were too big and didn't fit fully in the channel. Even if the guide is properly adjusted, tighten the nuts before going all the way to the back of the channel and pushing the guide up. A small change to a normal installation and everything worked fine.
