Came like in the photo. It's easy, I must say. Handles feel comfortable! It seems to be well glued there, in time we will see if it sticks. I'm afraid the size markings may wear off over time. So I have to take a sharp object and cut out the numbers. Came with stitch markers and needles as shown. Cover - smooth skin. Overall I am very pleased with my purchase. I will update later! Oh ps - the hook is tapered, not built in! Although I like Susan Bates hooks, they are smooth/fairly smooth for crocheting.
