Physically difficult to attach but otherwise easy to install. It does what it says on the tin and does a little more - I have a Volvo and the seat belts are awkward to put on and it actually makes things easier. They also don't allow the seat belt to tug or rub my neck on long drives - it doesn't make contact. I imagine it would have come loose in an accident with a lot of force, but the initial jerk force on your neck (carotid arteries and carotid arteries I think) would be a little less, and isn't that good? Thing? It's just a theory. If he survives the crash, great. I don't allow myself to be distracted by pressure and irritation in the neck.
