Hose exactly as pictured, no glued on Mubea style clamps like the old KM4542 (I have a 2012 Motorcraft KM4542 with hose clamps attached which is here). The hose must be secured at least on the plastic radiator neck with constant tension clamps. The cheapest are the Mubea clips (or Rotor-Clip or some other brands) compared to the more expensive stainless steel Bellevue spring clips like the Breeze (cheaper Breeze clips have carbon steel screws, not all stainless steel). Dimensions measured: 2.1" OD when mounted to the radiator, 2.5" OD when measuring the OD of the water pump at an angle. This measurement should be approximately the bottom center of your clamp, such as RotorClip CTB55/CTB61 or CTB67). Opinions vary on reusing old clamps, most people probably do - at the end of a radiator it's probably better to use classic worm clamps or prettier T-bolts than other non-constant tension alternatives.
