Bits don't fit well, corners chipped, cut well

Bit seems well balanced and cuts 1/32" deep spruce with no problems. But a tactical knife had a broken corner + edge. Also, the inserts don't fit well in the pocket (see circled gap in image). So the set screws are misaligned and I'm afraid the inserts might crack if I over tighten the screws. plastic container.
