Review on Chronic Care Management (CCM) Software by Jonathan Olson

I love how easy it was for my patients, who are older adults with dementia or Alzheimer’s disease in long term care facilities of different sizes throughout Canada including Quebec City where we have over 600 residents living here! The software does not work well at all when you first sign up but after some time they get used too them very quickly - which works great because our staff doesn't really understand computers so this helps us out tremendously!! We also like being able see their medication history as soon has its entered into CCMS by nursing team members since sometimes these records can be lost if there ever were any issues regarding drug interactions etc., This makes tracking much easier than having paper files around especially now during COVID19 pandemic due lockdown rules/restrictions. It would definitely help keep track better overall health status within facility or home setting without needing multiple.
