These parts came with oversized threads. They were cut with an M4 but the starter drill was much larger than it should have been. I had about 40-50 from this batch that just didn't work. You start tightening the bolt and it doesn't develop any real preload before it fails. I measured the hole diameter of some of them and they were in the 3.6-3.8mm range when they should be in the 3.3-3.5mm range. Luckily I had a few other T-Nuts left over from another project and was able to successfully complete the project. .
