Missized Handles in Power and Hand Tools

The handles were 5/16-18 threaded and would not fit 1/4-20 screws. Also, the hex bolt heads were slightly undersized and protruded beyond the T-rail. I had to go back. Curious how quality control resulted in wrong sized pens ending up in wrong packaging.
