As some readers have pointed out, once the holder is installed the paper towel roll is not flush with the cabinet or horizontal surface. The reason for this is that the manufacturer could not make the support cylinder anything smaller than the inside diameter of the paper towel roll. Since the detachable end (right side) is tapered, centering the paper roll is not a problem.
