I think the noise is due to the wear of the fan and the vibration of the Tivo's metal case. As the fan wears out, the vibration increases and the sound becomes LOUD. To make matters worse, the thermal paste between the heatsink and the CPU was old and dried up. This prevented the heatsink from cooling the processor.
